Labradoodles Puppy Application

We believe that our dogs deserve to live with families who will treat them with love, dignity and respect. We are very aware of the many animals who are given up because owners were not prepared for the commitment, expense and training time that goes in to raising a great pet. The following questions are asked to help us assess whether or not one of our labradoodles would fit in to your family life.

What is your name?

What is your email address?

What is your telephone number?

What city do you live in?   Province/State

What is your current housing status?

Do you have a fenced yard?

Why do you want a labradoodle?

Do all of your family members want a dog?

Have you had dogs before?

Have you ever had to give up a dog and what were the circumstances?

Are there other animals in the home?

Do you have children and if so what are the ages?

What are the daily work hours of the person primarily responsible for the pet?

Will you obedience train?

Where will the dog stay during the day?

How long will he/she be alone during the day?

Are you familiar with crate training and if so where would you put the crate?

What sex/color do you prefer?
